Exceptions need a DEC basis\nand fail when stale (Device urn:uuid: under DEC-008 clause 1 is the only one).\nMissing input fails closed. Eleven self-tests cover each rule negatively.\n\nThe first run on main reports 6 errors (ten_ in two forms and unregistered,\norg_/itm_/opr_ unregistered, fac_ not ULID) and 187 governed entities without\na prefix.
